Below you will find several forms that must be filled out and sent to MIMA prior to leaving the country. If you wish to donate or run a fundraiser, please click here

Here is a list of what you will need before you leave:

Faxing these documents to MIMA is fine. You should not fax the photocopy of your passport. You should keep one for yourself and mail the other to

The US is now requiring Yellow Fever Immunizations upon returning from Bolivia. These are difficult to find, since there is a shortage. Please contact your Health Dept ASAP or if you live in Jupiter, contact MIMA about going to Jupiter Hospital. They have forms that you can fill out in advance and then you'll be called when there is an immunization available.

Boliva is also requiring a visa ($100). We are working on trying to get these processed as a group. You will need the verification of your Yellow Fever Immunization when you apply for the visa.

We are going to a new hospital (a non-government hospital) and will be staying at the same hotel, Hotel Anteus. We have moved all the equipment to the new hospital, and it is ready to rock. We still need some monitors, portable if possible, and pulse oximeters.
